Class Members must legibly complete and submit this Claim Form to be eligible to receive monetary recovery. Your Claim Form must be postmarked and/or received on or before February 28, 2015, or it will be rejected.
Your Claim Form must be filled out completely and accurately, or it will be rejected.
If you do not want to participate in the Settlement, you may “opt out” of the Settlement by returning this Request for Exclusion Form. If you choose to opt out of the Settlement: (a) you will have no right to receive any money under the Settlement; (b) you will not be bound by the Settlement; and (c) you will have no right to object to the Settlement and/or be heard at the final approval hearing.
Deadline: Your Exclusion Form must be received by the Settlement Administrator by February 28, 2015, to be excluded from the settlement class.
